Long-Term Nursing Home Care
Placing a loved one in a long-term care setting can be the hardest decision to make for a family. Heartland Care Center has one-on-one meetings to understand the needs and cares of the individual to make the transition easier for everyone. With our homelike atmosphere and our family setting, we value our quality care and dedication of providing your love ones with our best care!
Skilled Nursing Care
Rehabilitation
At Heartland Care Center, we want the best for our residents. Many come to us for rehabilitation (skilled nursing care) that involves skilled therapy sessions with our 24-hour nursing care. If you are wondering what qualifications you must meet to be eligible for Medicare Part A therapies, feel free to give the facility a call. We would love to work with you or your loved ones to ensure we get you healthier, stronger, and maintain a comfortable lifestyle.
Assisted Living
Heartland Care Center offers a multi-level approach to senior living which provides the resident/tenant the seamless transition to a higher/lower level of care without having to leave the building. This is an easier transition as your loved one will have the same staff, seeing the same residents, and while maintaining our homelike atmosphere!
Our basic rate for Assisted Living includes a regular room and board, daily laundry, and three meals daily!
